Reports to: Program Director of Interfaith Works Residences

Employment Status: Full Time

FSLA Status: Exempt

Pay rate: 46,000-50,000





POSITION SUMMARY


Interfaith Works Residences (IWR) provides permanent supportive housing for 21 homeless gender-neutral clients using the Housing First and harm reduction models. The program is located on its own floor at Progress Place and designed to help highly vulnerable and chronically homeless individuals maintain housing in their own private living quarters with 24-hour support. Case management at IWR includes life skills development and tenancy support integrated with a variety of services at Progress Place and in the broader community. This is a full-time position.


ESSENTIAL DUTIES & RESPONSIBILITIES

The essential functions include, but are not limited to the following:


  1. Works with the Program Director and team of residential staff to ensure residents have the individualized support they need to maintain housing.
  1. Meets with clients weekly to identify needs and develop ongoing support in order to successfully maintain housing.
  2. Completes and updates client Assessment Tools.
  3. Develops a comprehensive Individual Service Plan with each client and documents client's progress toward each goal.
  4. Works with residents to develop life skills and provide residents with tenancy support aimed at helping them be good neighbors, maintain their units, keep a budget, and pay rent on time.
  5. Stays in communication with the property management company to work on tenancy issues and concerns with residents.
  6. Attends interdisciplinary team meetings with colleagues, including County agency workers, behavioral health and medical providers, and vocational workers to integrate all services and increase the likelihood of residents' success. Interdisciplinary meetings may also be necessary to respond to client crises and to establish preventive interventions.
  7. Updates client files regularly and enters information into the Homeless Management Information System (HMIS).
  8. Attends weekly individual supervision with the Program Director.
  9. Assists the Program Director in facilitating quarterly staff meetings, which may include in-service training.
  10. Join the Montgomery County's Homeless Continuum of Care Coordinated Entry Committee meetings held by the county.
  11. Join and participate in our bi-weekly Team meetings
  12. Accompany clients to a variety of appointments with LYFT when needed.
  13. Other duties as assigned.


MINIMUM QUALIFICATIONS (KNOWLEDGE, SKILLS, ABILITIES)


  1. Bachelor's degree in social work, counseling, or psychology is preferred, but commensurate experience will be considered.
  2. Two years direct service experience working with individuals who are homeless and have disabilities, which include mental illness, substance use disorders, medical conditions, and cognitive impairments.
  3. Understanding of trauma-informed care and the Housing First model.
  4. Experience with crisis intervention, including assessing for and responding to individuals with suicidal and homicidal ideation.
  5. Excellent interpersonal and communication skills to effectively work as part of a team.
  6. Ability to manage multiple responsibilities efficiently.
  7. Ability to work independently and with a team, collaborating with other Interfaith Works programs and departments.
  8. Excellent written and oral communication skills.
  9. Proficiency with Microsoft Windows and Office 365, in addition to Outlook, Word, Excel, and PowerPoint.
  10. Fluency in Spanish is a plus.
